Job Summary

We are seeking a highly skilled and motivated Physiotherapist to join our team at East Suffolk and North Essex NHS Foundation Trust. As a Senior Physiotherapist - Care Home Specialist, you will play a key role in shaping a new role that addresses the needs of care home residents, care homes, and commissioners, leading to positive change.

Main Responsibilities:

  • Lead Multidisciplinary Teams: Demonstrate experience in leading multidisciplinary teams to deliver high-quality care to adults in care homes.
  • Conduct Comprehensive Assessments: Conduct holistic patient assessments to identify individual needs and develop personalized care plans.
  • Build Strategic Partnerships: Build relationships with care home managers and owners to ensure seamless care coordination and improve outcomes.
  • Collaborate with Healthcare Providers: Collaborate with various healthcare providers to improve patient outcomes and reduce unnecessary hospital admissions.
  • Monitor and Evaluate: Produce monitoring reports to measure project effectiveness and identify areas for improvement.

Requirements:

  • Driving License: Possess a valid driving license and access to a vehicle.
  • Excellent Communication Skills: Demonstrate excellent communication skills to work effectively with care home staff, healthcare providers, and other stakeholders.
  • Strong Leadership Skills: Possess strong leadership skills to lead multidisciplinary teams and drive change.
