Job Summary

We are seeking a highly skilled and experienced Senior Physiotherapist to join our team in East Suffolk and North Essex NHS Foundation Trust. As a Senior Physiotherapist, you will play a key role in providing high-quality care and support to adults in care homes, ensuring they receive the right care in the right place.

Main Responsibilities
  • Provide holistic patient assessments to a range of patients, demonstrating excellent clinical skills and knowledge of relevant diagnoses and their management.
  • Work as an autonomous practitioner, building effective working relationships with patients, carers, and colleagues, and directing patient care.
  • Develop and gather information to demonstrate the effectiveness of the Care Home project, including the production of regular monitoring reports.
  • Maintain open communication with social care providers, primary care, and commissioning to make recommendations for improvements to support effective outcomes for adults.
  • Collaborate with the local Integrated Neighbourhood Team to ensure joined-up, seamless care.
Requirements
  • Proven post-registration experience in a health or social care setting.
  • Range of rotational acute/community care experiences, including acute discharge planning.
  • Demonstrable in-service training and reflective learning in these specialities.
  • Ability to work as an autonomous practitioner and build effective working relationships with patients, carers, and colleagues.
  • Registration with the Health and Care Professions Council (HCPC).
