Job Summary:

We are seeking a highly skilled and motivated Physiotherapist to join our team at East Suffolk and North Essex NHS Foundation Trust. As a Senior Physiotherapist - Care Home Specialist, you will play a key role in shaping a new role that addresses the needs of care home residents, care homes, and commissioners, leading to positive change.

Main Responsibilities:

  • Lead Multidisciplinary Teams: Demonstrate experience in leading multidisciplinary teams to deliver high-quality care to adults in care homes.
  • Holistic Patient Assessments: Conduct comprehensive holistic patient assessments to identify individual needs and develop personalized care plans.
  • Care Home Partnerships: Build strong relationships with care home managers and owners to ensure seamless care coordination and improve outcomes.
  • Collaboration and Communication: Collaborate with various healthcare providers to improve patient outcomes and ensure effective communication across teams.
  • Project Evaluation: Produce monitoring reports to measure project effectiveness and inform future service development.

Requirements:

  • Driving License: Possess a valid driving license and access to a vehicle to facilitate travel between care homes.
  • Professional Qualifications: Hold relevant professional qualifications and certifications in physiotherapy.
