About the Role:

We are seeking a dedicated Care Coordinator to join our team at Cera Care Ltd. In this role, you will be responsible for managing the delivery of high-quality care services. Your main tasks will include matching Care Workers to Clients based on their specific care needs and ensuring proximity whenever possible.

Key Responsibilities:
  • Develop and implement effective care plans to meet the needs of our Clients
  • Conduct thorough assessments and reviews to ensure the best possible care outcomes
  • Collaborate with Care Workers to ensure seamless delivery of care services
  • Monitor and evaluate the quality of care services to identify areas for improvement
  • Stay up-to-date with relevant regulations and compliance requirements
Requirements:
  • NVQ/SVQ 2 or 3 qualification preferred
  • SSSC registration
  • Minimum of 2 years experience in care coordination
  • Knowledge of CQC/CI/CIW Compliance
  • Excellent communication and interpersonal skills
  • Organized, detail-oriented, and flexible
  • Ability to work as part of a team
