Position Overview

The Lowestoft Primary Care Network (PCN) is dedicated to serving a diverse patient population of around 80,000 across multiple healthcare practices. The Patient Care Facilitator will play a crucial role in supporting the practice and the PCN, operating within established professional and clinical frameworks as part of a collaborative multi-disciplinary team. This position focuses on providing timely and personalized care for patients, particularly those with chronic health conditions, complex health needs, or recent cancer diagnoses. The role will contribute to enhancing cancer care quality by assisting the practice and PCN in refining their processes, achieving set objectives, and ensuring patients receive appropriate support throughout their healthcare journey.

The Patient Care Facilitator is expected to collaborate with fellow care facilitators within the Lowestoft network to ensure comprehensive support for all patients. The successful candidate should be adaptable to work across various practices within the network, with primary responsibilities based at Bridge Road Surgery.

Key Responsibilities

The Patient Care Facilitator will assist patients in preparing for or following up on discussions with healthcare professionals. This role involves close collaboration with general practitioners and other primary care staff to guarantee that patients receive tailored care and support for effective health management. Key responsibilities include:

  • Collecting relevant health information to support patient review appointments.
  • Creating personalized care plans and coordinating with Social Prescribing Link Workers to facilitate access to community-based non-clinical support.
  • Engaging with patients through various communication methods, including electronic systems, letters, phone calls, video consultations, or in-person meetings.

As part of the role, the Patient Care Facilitator will also serve as a cancer care coordinator, responsible for organizing monthly cancer care meetings and managing administrative tasks related to end-of-life care, while maintaining communication with patients and the community palliative care team. This includes conducting audit searches and ensuring accurate data collection.

Additionally, the facilitator will be involved in triaging, directing, and navigating patients within the evolving capacity and access initiatives undertaken by the PCN.
