Taurus Healthcare Limited is seeking a dedicated and passionate Health Improvement Practitioner to join our team. In this role, you will play a vital part in empowering individuals and communities to improve their health and wellbeing.

As a Health Improvement Practitioner, you will work closely with patients, families, and carers to understand their needs and support them in making positive lifestyle changes. You will utilize your knowledge of public health issues, health inequalities, and the determinants of health to deliver effective interventions and promote healthy behaviours.

Responsibilities
  • Collaborate with patients and communities to identify health improvement needs and develop tailored support plans.
  • Provide motivational coaching, behavioural change techniques, and goal-setting guidance to individuals seeking to improve their health.
  • Collect and analyze data to measure the impact of health improvement interventions and contribute to service evaluation.
  • Work in partnership with other healthcare professionals, community organizations, and stakeholders to deliver integrated health services.
  • Promote awareness of public health issues and resources within the community.
Qualifications & Experience

Essential Requirements:

  • Educated to GCSE or equivalent level.
  • Demonstrable commitment to professional and personal development.
  • Experience of data collection and using tools to measure the impact of services, such as the Patient Activation Measure.
  • Experience in health promotion and health improvement.
  • Knowledge of the personalised care approach and primary care networks.
  • Understanding of the determinants of health, including social, economic, and environmental factors.
  • Knowledge of public health issues and health inequalities within Herefordshire.
  • Commitment to equality, diversity, and inclusion.
  • Knowledge of IT systems, including word processing skills, email, and internet usage.
  • Full Driving License.

Desirable Requirements:

  • NVQ 3 or equivalent and/or relevant basic/first level professional qualification or working towards this.
  • NHSE Approved training in motivational coaching, behavioural change, and goal setting.
  • Experience of working directly in a community development context, adult health and social care, learning support, or public health/health improvement (can include unpaid work).
  • Experience of supporting people, their families, and carers to change health behaviours and improve wellbeing (can include unpaid work).
  • Experience of supporting people with their mental health, either in a paid, unpaid, or informal capacity.
  • Experience of partnership/collaborative working and building relationships across a variety of organisations, including the voluntary sector.
  • Experience using clinical systems, EMIS.
  • Knowledge of the needs of patients with long-term conditions.
  • Knowledge of community development approaches and local programmes of work, Talk Community.
