Position Overview: As a key member of the multi-agency Health & Justice team, you will play a vital role in supporting individuals within the Health & Justice demographic. Your responsibilities will include:

Key Responsibilities:
1. **Referral Management:** Receive and process referrals for individuals in the Health & Justice population.
2. **Outreach Engagement:** Collaborate with the outreach team to assess the unique needs of service users and their families.
3. **Service Coordination:** Provide liaison and joint-working opportunities, facilitating referrals to appropriate services.
4. **Vulnerability Screening:** Conduct both face-to-face and electronic screenings to identify various vulnerabilities among individuals who have interacted with the criminal justice system or immigration centers.
5. **Guideline Adherence:** Work within established guidelines and recognize when to escalate issues to Practitioners.
6. **Support Roles:** Assist Practitioners, STR Workers, Peer Support Workers, and Administrative staff as needed.

Qualifications: The ideal candidate will possess a strong understanding of the challenges faced by individuals in the Health & Justice sector and demonstrate the ability to work effectively within a team environment. A commitment to supporting vulnerable populations is essential.
