We are seeking a highly skilled and experienced Advanced Clinical Practitioner to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to patients with acute mental health needs.
Key Responsibilities- Lead clinical discussions and meetings to ensure effective care planning and delivery.
- Develop and implement treatment plans in collaboration with patients, families, and other healthcare professionals.
- Conduct comprehensive assessments and reviews to inform care decisions.
- Provide clinical supervision and support to junior colleagues.
- Collaborate with other teams and services to ensure seamless care transitions and coordination.
- Participate in quality improvement initiatives and contribute to the development of clinical policies and procedures.
- Registered Nurse, Allied Health Professional, Paramedic, or Pharmacist with a degree-level qualification.
- Experience in acute mental health settings, with a focus on working with patients with complex needs.
- Non-Medical Prescribing qualification or equivalent experience.
- Strong leadership and communication skills, with the ability to work effectively in a fast-paced environment.
