Manchester University NHS Foundation Trust is seeking a highly motivated and experienced Community Specialist Practitioner (CSP) to join our dynamic District Nursing Team in south Manchester.

As a CSP, you will play a pivotal role in leading and coordinating nursing teams to deliver complex care to adults within their homes and various community settings. This position offers an exciting opportunity to make a real difference in the lives of patients while working collaboratively with a dedicated team of healthcare professionals.

Key Responsibilities:
  • Provide expert nursing care to adults in diverse community settings, including homes and other locations.
  • Lead and supervise a team of nurses, ensuring effective delivery of patient-centered care.
  • Collaborate closely with healthcare partners, including GPs, social workers, and therapists, to develop comprehensive care plans.
  • Participate in clinical audits and quality improvement initiatives to enhance the delivery of nursing services.
  • Demonstrate strong communication and interpersonal skills to effectively interact with patients, families, and colleagues.
Essential Requirements:
  • Registered Nurse with a valid NMC registration.
  • Community Specialist Practitioner qualification.
  • Proven experience in leading and managing nursing teams.
  • Excellent clinical skills and knowledge of best practices in community care.
  • Strong communication, interpersonal, and organizational skills.
