Job Summary:

We are seeking an experienced Community Nursing Specialist Practitioner to join our team in North Trafford. As a key member of our Community Nursing Service, you will be responsible for providing high-quality nursing care to patients in their own homes and in various community settings.

Key Responsibilities:

  • Lead and coordinate nursing teams to provide complex nursing care to adults in their own homes and in community settings
  • Work closely with the Multi-Disciplinary Team (MDT) to ensure effective patient care and coordination
  • Develop and implement individualized care plans to meet the unique needs of patients
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes
  • Contribute to the development and implementation of nursing policies and procedures

Requirements:

  • Relevant CSP qualification and experience in community nursing
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
  • Valid full driving license and access to a car
