About the Role

We are seeking a highly skilled and experienced District Nurse to join our Community Nursing Team in Manchester. As a key member of our team, you will be responsible for providing high-quality nursing care to patients in their own homes and in various community settings.

Key Responsibilities
  • Lead and coordinate nursing teams to deliver complex nursing care to adults in their own homes and in community settings.
  • Work closely with multidisciplinary teams, including social workers, GPs, and allied health professionals, to ensure effective patient care.
  • Develop and implement clinical skills and knowledge to improve patient outcomes.
  • Provide leadership and mentorship to junior members of staff.
  • Contribute to the development of the Community Nursing Service and participate in quality improvement initiatives.
Requirements
  • Registered Nurse with a Community Specialist Practitioner qualification.
  • Experience of working in community nursing and leading teams.
  • Strong clinical skills and knowledge of community nursing practices.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
