Transforming Patient Care in the Community
We are seeking an experienced Advanced Clinical Practitioner to join our Community Response Team, where you will play a key role in delivering high-quality patient care and supporting the development of our Hospital at Home service.
As a member of our team, you will work closely with our multidisciplinary team to ensure safe and effective discharges, prevent re-admissions, and provide education and support to colleagues as needed.
Main Responsibilities- Lead Clinical Practice: Provide medical assessments at home, working alongside GPs and under the guidance of hospital consultants, to ensure patients receive safe and effective treatment.
- Service Development: Contribute to the development of new care pathways and service improvements, leveraging your expertise to enhance patient outcomes and experience.
- Collaboration and Communication: Foster strong relationships with acute and community colleagues, using your specialist clinical skills to determine the most appropriate pathways for patients and ensuring seamless transitions between services.
- Education and Support: Provide education and support to colleagues, promoting quality improvement initiatives and service development within the team.

Person Specification- Qualifications: MSC in Advanced Clinical Practice, relevant professional health degree, independent non-medical prescriber (V300) qualification, and current registration with NMC or HPC.
- Experience: Proven experience of working as an independent practitioner, teaching and mentoring staff, and caseload management.
- Personal Qualities: Self-motivated, innovative, assertive, empathetic, and flexible, with the ability to manage and diffuse stressful situations.
