Job Overview

The Advanced Nurse Practitioner will operate within their professional scope to deliver comprehensive patient care, encompassing initial triage, thorough history taking, clinical evaluation, diagnosis, treatment, and ongoing assessment of care both in the clinic and during home visits. This role requires critical thinking and sound clinical judgment throughout the decision-making process.

The position is designed to enhance our current team of skilled Practice Nurses and Healthcare Assistants who manage essential nursing responsibilities, health promotion, chronic disease management, and minor illness evaluations. The successful candidate will work independently, conducting face-to-face clinical assessments during scheduled appointments, making informed clinical decisions based on their expertise and analytical skills.

The ideal applicant must be a qualified Nurse Prescriber, willing to conduct home visits, make necessary clinical referrals, and manage pathology results and hospital communications for their patients.

Main Responsibilities:

  • Assist with daily operational duties
  • Prescribe and provide guidance on medications
  • Address minor injuries and illnesses
  • Conduct triage assessments
  • Perform comprehensive history taking
  • Demonstrate the ability to work autonomously and effectively under pressure
  • Exhibit leadership qualities
  • Participate in Practice Meetings
  • Conduct audits of nursing practices
  • Refer patients to appropriate healthcare professionals within the Primary Care Team and external agencies, involving GPs as necessary
  • Collaborate as part of a coordinated nursing team and engage in joint initiatives
  • Attend training sessions to maintain professional qualifications and expertise
  • Commit to ongoing professional development
