Position Overview

Greenbank Surgery is seeking a skilled Clinical Nurse Specialist to become a vital member of our supportive, innovative, and collaborative healthcare team.

As a reputable practice with a strong track record in quality care, we serve a diverse patient population of approximately 8400 individuals.

The ideal candidate will possess a comprehensive understanding of Primary Care, enabling them to deliver exceptional nursing services to our patient community while adhering to their professional scope of practice.

Familiarity with TPP SystmOne is advantageous.

Work Schedule

This position requires a commitment of 25 hours per week, Monday through Friday.

Key Responsibilities

The Clinical Nurse Specialist will collaborate with the nursing team, which includes both nurses and healthcare assistants. As part of a multidisciplinary team, the successful candidate will ensure that nursing services are provided effectively to our patient population. Responsibilities include:

  • Leading initiatives in infection prevention and control, health promotion, and chronic disease management.
  • Conducting well-woman and well-man clinics, while supporting the management team in the evaluation and implementation of clinical policies and procedures.

Core Responsibilities

The following are the primary duties expected of the Clinical Nurse Specialist. Additional tasks may be assigned based on workload and staffing needs:

  • Act as the lead for Infection Prevention Control (IPC), ensuring compliance with IPC policies through audits and training.
  • Develop and implement health promotion programs aimed at enhancing patient well-being.
  • Manage and support patients at risk of developing chronic conditions, focusing on preventive care.
  • Provide routine nursing care in accordance with clinical guidelines and best practices.
  • Oversee chronic disease clinics and coordinate care for patients requiring specialist referrals.
  • Maintain accurate clinical records and ensure effective communication within the nursing team.

Qualifications and Skills

Essential:

  • Registered Nurse with experience in a primary care setting.
  • Strong clinical skills in areas such as wound care, chronic disease management, and health promotion.
  • Excellent communication and interpersonal skills.
  • Ability to work both independently and as part of a team.

Desirable:

  • Experience with SystmOne and clinical audit processes.
  • Postgraduate qualifications in Chronic Disease Management.
