Overview of the Role

Folly Lane Medical Centre is a welcoming and dedicated healthcare facility committed to serving a diverse community. Our team is passionate about providing outstanding patient care, and we are currently seeking a Clinical Nurse Specialist to join us for 20 hours each week, specifically on Monday, Wednesday, and Friday. The successful candidate will play a crucial role in delivering comprehensive healthcare services to our patient population. This position emphasizes evidence-based practices for managing long-term conditions and promoting preventive health measures.

Key Responsibilities

The main responsibilities of this role include:

  • Management of chronic health conditions
  • Conducting assessments and developing treatment plans for patients with long-term illnesses
  • Implementing safe healthcare practices and monitoring equipment
  • Administering routine immunizations and assisting in emergency situations
  • Providing both in-person and telehealth consultations
  • Performing cervical smears and supporting treatment room activities

About Our Practice

We are a progressive training practice with a patient list exceeding 10,200 individuals. Our team is supportive and fosters a positive working environment. Our clinical staff comprises:

  • 5 GP partners
  • 3 Salaried GPs
  • GP trainees
  • Nursing team including Nurse Practitioners and Healthcare Assistants
  • Administrative and Reception staff
  • Mental health professionals
  • Additional support staff such as Clinical Pharmacists and Social Prescribers

Responsibilities of the Role

  • Assess, plan, and evaluate health programs that enhance patient well-being.
  • Collaborate with healthcare professionals to manage long-term conditions effectively.
  • Provide education and support to patients regarding medication and treatment adherence.
  • Implement vaccination programs for both adults and children.
  • Conduct holistic assessments for patients requiring cervical cytology and wound care.
  • Maintain confidentiality and adhere to clinical guidelines and protocols.

Required Qualifications

Essential

  • Registered General Nurse qualifications/certificates
  • Experience in primary care settings
  • Strong understanding of chronic disease management
  • Commitment to ongoing professional development

Desirable

  • Familiarity with IT clinical systems
  • Knowledge of Microsoft Office applications
