Job Overview
The Nurse Practitioner will operate within their professional scope to deliver comprehensive patient care, including initial assessment, clinical evaluation, diagnosis, treatment, and ongoing care management. This role requires critical thinking and the ability to make informed clinical decisions. The Nurse Practitioner will work collaboratively as a vital member of the nursing team and the broader multidisciplinary practice team, enhancing efficiency and skill utilization.
This position is designed to augment our existing team of dedicated Practice Nurses and Healthcare Assistants who perform essential nursing duties, health promotion, chronic disease management, and minor illness evaluations. The successful candidate will be expected to work independently, conducting face-to-face clinical assessments during scheduled appointments, and making clinical judgments based on their expertise and analytical skills regarding undiagnosed conditions. Familiarity with SystmOne clinical software is advantageous.
The candidate must be a qualified Nurse Prescriber, willing to conduct home visits, make appropriate clinical referrals, and manage pathology results and hospital communications for patients.
Main Responsibilities:
- Support daily operational duties
- Prescribe and provide guidance on medications
- Manage minor injuries and illnesses
- Triage patients effectively
- Conduct thorough history taking
- Work autonomously and under pressure
- Exhibit leadership qualities
- Participate in Practice Meetings
- Conduct audits of nursing practice
- Refer patients to appropriate services within the Primary Care Team and external agencies, with GP involvement as necessary
- Collaborate with a coordinated nursing team and engage in joint initiatives
- Attend training to maintain professional qualifications and expertise
- Demonstrate a commitment to continuous professional development
