Supported Care Practitioner: £26,421 to £28,770 per annum (Scale F)

The Social Care Community Engagement Team operates as a telephone-based service, leading the charge in implementing the 'Living Well' initiative, which represents Norfolk's approach to the 3 Conversations model—a proven method for enhancing Social Care delivery. This initiative emphasizes the importance of strengthening our initial contact points, enabling social care professionals to engage in meaningful dialogues with individuals to foster improved outcomes. It eliminates process-driven 'hand-offs' and promotes connections with local networks and community support systems.

Candidates should possess exceptional communication abilities, proficient record-keeping, and keyboard skills, along with experience in supporting vulnerable individuals in need of care. A strong commitment to outstanding customer service and the capability to manage a substantial workload in an organized manner are essential. You will be expected to collaborate effectively within a multidisciplinary framework and be well-versed in the department's eligibility criteria. Regular individual supervision, peer support, and access to relevant training will be provided to facilitate your professional growth.

Depending on your qualifications and experience, you may be appointed as a Supported Care Practitioner (Grade F) , with the expectation to complete our Assistant Practitioner Development Programme within an agreed timeframe. This program is designed to enhance your understanding of the role and further develop your skills and knowledge. Successful completion will allow for progression to the role of Assistant Practitioner (Grade G) .
