The Social Care Community Engagement Team operates as a telephone-based service that plays a pivotal role in executing the 'Living Well' initiative, which is Norfolk's approach to the 3 Conversations model—a proven strategy for transforming Social Care delivery. The 'Living Well' initiative emphasizes the importance of enhancing our initial contact processes, enabling social care professionals to engage in meaningful dialogues with individuals to foster improved outcomes. This approach eliminates the traditional, process-driven 'hand-offs' and promotes collaboration among professionals to connect with local networks and community support systems.

Candidates should possess exceptional communication abilities, proficient record-keeping, and keyboard skills, alongside experience in assisting vulnerable individuals in need of care and support. A strong commitment to outstanding customer service and the capability to manage a high volume of work efficiently and in an organized manner are essential. Additionally, a dedication to interdisciplinary collaboration and familiarity with departmental eligibility criteria is required. Regular individual supervision, peer support, and access to relevant training will be provided to facilitate your professional growth and development.

Depending on your qualifications and experience, you may be considered for the role of Supported Care Practitioner (Grade F) , with the expectation to complete our ' Assistant Practitioner Development Program' within an agreed timeframe. This program is designed to support you in mastering your responsibilities and further developing your knowledge and skills. Successful completion of the program will allow for progression to the role of Assistant Practitioner (Grade G).
