Clinical Coordinator RN

We are seeking a highly skilled and compassionate Clinical Coordinator RN to join our team at KDI Knickerbocker Dialysis, Inc. As a Clinical Coordinator RN, you will play a vital role in ensuring the highest level of care for our patients with end-stage renal disease (ESRD) or chronic kidney disease.
About the RoleAs a Clinical Coordinator RN, you will be responsible for coordinating patient care and services, including trending lab values, tracking vital signs, and managing outcomes. You will work closely with our interdisciplinary team to ensure the safety, comfort, and well-being of our patients.
Responsibilities- Coordinate patient care and services, including trending lab values and tracking vital signs
- Manage patient outcomes and ensure the highest level of care
- Work closely with the interdisciplinary team to ensure patient safety and comfort
- Develop and implement care plans to meet the unique needs of each patient
- Collaborate with other healthcare professionals to ensure seamless care transitions
- Current Registered Nurse (RN) license in the state of practice
- At least 18 months of registered nursing experience
- Dialysis experience preferred
- Basic computer skills and proficiency in MS Word and Outlook required
