Job Summary

The Role

This is an exciting opportunity to work with individuals who may be at risk of homelessness and have experienced housing challenges, with substance misuse, alcohol, and mental health difficulties as part of a multi-disciplinary team. Our team provides a non-judgmental, recovery-based approach that values the strengths and choices of those individuals.

We are seeking a skilled and compassionate Outreach Practitioner to join our team. As a key member of our service, you will work closely with our clinical team, counsellors, and other professionals to provide excellent quality of care to people who will be using our service to access treatment for substance use and their complex needs.

Key Responsibilities

  • Manage a caseload of individuals, including prescribing and providing interventions at varying stages of their recovery
  • Develop and deliver a wide range of groups to support individuals in their recovery journey
  • Work collaboratively with our clinical team and other professionals to provide comprehensive care
  • Contribute to the development and implementation of service policies and procedures

The Service

Our service provides a partnership-based approach to substance use treatment, working closely with local authorities and other organizations to provide a comprehensive range of services.
