About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our Community Assertive Outreach Team at Pullman Place. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care and support to working-age adults with severe and enduring mental health problems.

Key Responsibilities
  • Conduct comprehensive health assessments and develop individualized care plans in partnership with service users and their carers.
  • Provide specialist care, education, and advice to service users, including those at risk, and coordinate care through the Care Programme Approach.
  • Take responsibility for the supervision and appraisal of team members, including induction of junior staff.
  • Ensure timely and effective clinical and managerial supervision, focusing on quality of care and audit.
  • Develop and implement care plans in conjunction with service users, carers, and other professionals, reflecting individual needs and promoting recovery and social inclusion.
  • Implement risk assessments and management plans on a daily basis.
Requirements

To be successful in this role, you will require:

  • Experience in mental health, with a strong understanding of serious mental illness.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage competing demands.
  • A commitment to delivering high-quality care and support, with a focus on promoting recovery and social inclusion.
