About the Role

We are seeking a skilled and compassionate Senior Rehabilitation Assistant to join our team at Calderdale and Huddersfield NHS Foundation Trust. As a key member of our multi-disciplinary team, you will play a vital role in providing high-quality care and support to adults of all ages who require rehabilitation and re-ablement services.

Key Responsibilities
  • Implement treatment plans and interventions to support individuals in maintaining their independence and dignity.
  • Work collaboratively with registered practitioners and other healthcare professionals to deliver comprehensive care and support.
  • Assist with the assessment and implementation of mobility and movement programs to restore optimum movement and functional independence.
  • Deliver exercise sessions to improve health and wellbeing, and provide information and advice on healthy eating and nutrition.
  • Contribute to the discharge of individuals into the care of another service, and provide support and guidance to carers and family members.
Requirements
  • GCSE Level English and Maths or equivalent.
  • NVQ L3 or equivalent experience.
  • Willingness to participate in competency work-based training programs and undertake additional and mandatory training as required.
  • Recent relevant experience working and communicating with the public in social care/healthcare, and experience of dealing with adult clients with complex needs and their carers.
  • IT literacy and an understanding of the needs and issues concerning illness and disability.
