Cornwall Partnership NHS Foundation Trust is seeking a dedicated and passionate Senior Community Mental Health Practitioner to join our Integrated Community Mental Health Team in Redruth.

Your Responsibilities

  • Conduct comprehensive assessments of patient needs and develop individualized treatment plans.
  • Provide direct clinical care and support to a designated caseload of patients.
  • Collaborate with multidisciplinary teams, including primary and secondary care providers, to ensure seamless and coordinated care.
  • Supervise and mentor junior staff members, fostering a culture of professional development and excellence.
  • Stay abreast of current mental health best practices and contribute to service improvement initiatives.

Qualifications & Experience

  • Current professional registration with the HCPC (RMNC).
  • Post-basic training in mental health nursing, social work, or occupational therapy.
  • Extensive experience working with individuals experiencing mental health challenges.
  • Demonstrated clinical leadership skills and a commitment to quality care.
