About the Role
We are seeking a highly motivated and experienced Dementia Practitioner to join our team at Cornwall Partnership NHS Foundation Trust. As a Dementia Practitioner, you will play a key role in providing specialist support to individuals living with dementia and their families, working closely with general medical colleagues to ensure the physical and mental health needs of people with dementia are considered.
Main Responsibilities
Key responsibilities of the role include:
- Providing specialist support to individuals living with dementia and their families
- Working in partnership with general medical colleagues to ensure the physical and mental health needs of people with dementia are considered
- Identifying individuals with dementia living in the community who are not currently recorded on the GP dementia registers and improving GP awareness and detection of dementia
- Working with Complex Care and Dementia and memory assessment service teams to provide a comprehensive service across the pathway for people diagnosed with dementia
