About the Role

We are seeking a highly skilled and experienced Patient Flow Coordinator to join our team at Cornwall Partnership NHS Foundation Trust. As a key member of our multi-disciplinary team, you will play a pivotal role in supporting the patient journey through mental health inpatient services.

Key Responsibilities
  • Supporting professionals, carers, and patients to develop safe and sustainable discharge plans.
  • Actively participating as a key member of the multi-disciplinary team.
  • Managing a caseload as delegated by supervisor.
  • Working closely with bed coordinators to support the flow through the mental health wards.

Requirements
  • Experience of safe and timely discharge.
  • Demonstrable knowledge of recovery approaches, supporting daily living skills, and working with vulnerable adults.
