About the Role

We are seeking a skilled and dedicated individual to join our team as a Patient Flow Coordinator. This is an exciting opportunity to make a real difference in the lives of our patients and their families.

Key Responsibilities

The successful candidate will be responsible for supporting the patient journey through mental health inpatient services, ensuring timely and seamless transfer of care on discharge. This will involve working closely with the multi-disciplinary team to develop safe and sustainable discharge plans, managing a caseload as delegated by supervisor, and collaborating with bed coordinators to support the flow through the mental health wards.

Person Specification

We are looking for an individual with experience of safe and timely discharge, as well as demonstrable knowledge of recovery approaches, supporting daily living skills, and working with vulnerable adults. A desirable candidate will also have experience of supervising/managing people and awareness of local services.
