Position Overview

We are pleased to announce a rewarding opportunity within the Manchester University NHS Foundation Trust for a Palliative Care Support Practitioner.

This role involves working collaboratively within a dedicated Palliative Care Team, ensuring the delivery of a well-coordinated and comprehensive service for individuals receiving specialized palliative care, including those residing in care facilities across the region.

The successful candidate will be an integral part of a multi-disciplinary team that includes Medical Professionals, Nurses, Dietitians, Occupational Therapists, Physiotherapists, and Speech and Language Therapists. The team's mission is to provide high-quality palliative care and support rehabilitation for patients diagnosed with cancer, regardless of the stage of their illness.

Key Responsibilities

The Palliative Care Support Practitioner will collaborate with the multi-disciplinary team, which consists of Medical Professionals, Dietitians, Occupational Therapists, Physiotherapists, Nurses, and Speech and Language Therapists. The primary goal is to deliver effective rehabilitation support and/or palliative care to patients with life-limiting conditions requiring specialized intervention.

The role includes assisting other healthcare professionals in the implementation, delivery, and assessment of rehabilitation and educational programs aimed at promoting independence and enhancing health, functional abilities, and overall quality of life.

The practitioner will manage a small caseload under the guidance of the supervising clinician, aiding patients with life-limiting illnesses to optimize their health and facilitate care in their preferred environment.

Additionally, candidates must demonstrate the ability to travel within the community as part of their role.
