Macmillan Assistant Practitioner/Nursing Associate

We are seeking an experienced and skilled Assistant Practitioner/Nurse Associate to join our Macmillan Specialist Palliative Care Team. As a key member of our team, you will contribute to the provision of a coordinated and comprehensive service for patients receiving specialist palliative care.
Key Responsibilities- Work as part of the Macmillan Specialist multi-disciplinary team to provide effective support rehabilitation and/or palliative care to patients with a life-limiting condition.
- Assist other professionals in the implementation, delivery, and evaluation of rehabilitation and education programs to promote independence and encourage improved health, functional capabilities, and quality of life.
- Hold a small caseload under the supervision of the responsible clinician, supporting patients with life-limiting illness to maximize their health and enable them to be cared for in their preferred place.
- Demonstrate the ability to be mobile within the community.

Requirements- Foundation degree or equivalent qualification.
- Experience working in rehabilitation or community settings.
- Good communication skills and computer literacy.
