We are seeking a highly skilled and experienced Palliative Care Specialist to join our team at St Margaret of Scotland Hospice. The successful candidate will have a strong background in Palliative Care and will be able to work effectively in a community and in-patient setting.

Key Responsibilities

  • To work closely with members of the multi-professional team within St Margaret of Scotland Hospice and the wider community projects/services to provide specialist palliative care within the Hospice and community setting.
  • To participate in daily Ward Rounds, weekly Grand Ward Rounds and daily Patient handovers within the Specialist Palliative Care Centre St Josephs Ward.
  • To establish oneself as a resource, facilitator and role model for Specialist Palliative Care within the Hospice in-patient and community settings.
  • To adopt a multi-professional and collaborative team approach to care assessment and delivery to ensure the needs of community-based patients and families are met.
  • To participate in the Hospice Virtual Clinic and/or deputise for the Community Palliative Care Lead as and when required.
  • To liaise with the Hospice Social Worker to optimise the psychosocial aspects of the patient and family from a quality-of-life perspective.
  • To liaise with the Hospice Therapy Team to optimise the functional aspects of the patient and family from a quality-of-life perspective.
  • To support families/carers in bereavement, referring appropriately to child/young person and adult Bereavement Services, should long-term support be required.
