About the Role

CV-Library is seeking a highly skilled and compassionate Clinical Case Manager to join our team. As a Clinical Case Manager, you will play a vital role in supporting children and young people with complex injuries, including traumatic brain injury, acquired brain injury, spinal cord injury, and amputation.

Key Responsibilities

  • Develop and implement client-centred rehabilitation packages to support children and young people in achieving their goals and maximizing their potential.
  • Work collaboratively with multidisciplinary teams, including healthcare professionals, support workers, and families, to ensure comprehensive care and support.
  • Conduct regular face-to-face meetings with clients and their families to assess their needs and progress.
  • Provide expert advice and guidance on rehabilitation strategies and techniques to support children and young people in achieving their goals.
  • Maintain accurate and detailed records of client progress and outcomes.
  • Participate in regular training and professional development to stay up-to-date with best practices and industry developments.

Requirements

  • Background in Occupational Therapy, Nursing, Physiotherapy, Speech and Language Therapy, Social Work, or Clinical Psychology.
  • Registration with HCPC, NMC, or Social Work England.
  • Experience working with children and young people in a clinical capacity.
  • Ability to demonstrate an understanding of child development and the needs of children and young people with complex injuries.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.

Desirable Skills

  • Experience in a similar case management role.
  • Knowledge of cloud-based case management and reporting systems.
  • Advanced membership of professional organizations, such as BABICM.
  • Experience in care planning and managing care teams.
