Clinical Case Manager

About the Role
CV-Library is seeking a highly skilled and compassionate Clinical Case Manager to join our team. As a Clinical Case Manager, you will play a vital role in supporting children and young people with complex injuries, including traumatic brain injury, acquired brain injury, spinal cord injury, and amputation.
Key Responsibilities
- Develop and implement client-centred rehabilitation packages to support children and young people in achieving their goals and maximizing their potential.
- Work collaboratively with multidisciplinary teams, including healthcare professionals, support workers, and families, to ensure comprehensive care and support.
- Conduct regular face-to-face meetings with clients and their families to assess their needs and progress.
- Provide expert advice and guidance on rehabilitation strategies and techniques to support children and young people in achieving their goals.
- Maintain accurate and detailed records of client progress and outcomes.
- Participate in regular training and professional development to stay up-to-date with best practices and industry developments.
Requirements
- Background in Occupational Therapy, Nursing, Physiotherapy, Speech and Language Therapy, Social Work, or Clinical Psychology.
- Registration with HCPC, NMC, or Social Work England.
- Experience working with children and young people in a clinical capacity.
- Ability to demonstrate an understanding of child development and the needs of children and young people with complex injuries.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organizational and time management skills.
Desirable Skills
- Experience in a similar case management role.
- Knowledge of cloud-based case management and reporting systems.
- Advanced membership of professional organizations, such as BABICM.
- Experience in care planning and managing care teams.
