Key Responsibilities:

  • Develop and implement effective case management plans to support clients with catastrophic and complex injuries.
  • Collaborate with healthcare professionals, families, and other stakeholders to ensure comprehensive care and support.
  • Conduct thorough assessments and evaluations to identify client needs and develop personalized plans.
  • Monitor and review client progress, making adjustments to plans as needed.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.

Requirements:

  • Proven experience in case management, preferably in catastrophic and complex injuries.
  • Strong knowledge of case management principles, practices, and regulations.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team.
  • Strong analytical and organizational skills.
