We are seeking a highly skilled and dynamic Clinical Case Manager to join our Children and Young People team. As a Clinical Case Manager, you will provide support to children and young people with a range of needs, including traumatic brain injury, acquired brain injury, spinal cord injury, and amputation.
Key Responsibilities- Develop client-centred rehabilitation packages
- Provide support to children and young people in a clinical capacity
- Work with multi-disciplinary teams to deliver high-quality care
- Manage and supervise clinical and non-clinical services
- Maintain accurate records and reports
- Background in Occupational Therapy, Nursing, Physiotherapy, Speech and Language Therapy, Social Work, or Clinical Psychology
- Registration with HCPC, NMC, or Social Work England
- Experience working with children and young people in a clinical capacity
- Ability to support children and young people to achieve the best outcomes
- Proactive and able to work independently
- Excellent organisational, communication, and computer skills
- Experience in a similar case management role
- Knowledge of iinsight, a cloud-based case management and reporting system
- Advanced BABICM membership
- Experience in care planning and managing care teams
