About the Role

We are seeking a highly skilled and dynamic Clinical Case Manager to join our Children and Young People team. As a Clinical Case Manager, you will provide support to children and young people with a range of needs, including traumatic brain injury, acquired brain injury, spinal cord injury, and amputation.

Key Responsibilities
  • Develop client-centred rehabilitation packages
  • Provide support to children and young people in a clinical capacity
  • Work with multi-disciplinary teams to deliver high-quality care
  • Manage and supervise clinical and non-clinical services
  • Maintain accurate records and reports
Requirements
  • Background in Occupational Therapy, Nursing, Physiotherapy, Speech and Language Therapy, Social Work, or Clinical Psychology
  • Registration with HCPC, NMC, or Social Work England
  • Experience working with children and young people in a clinical capacity
  • Ability to support children and young people to achieve the best outcomes
  • Proactive and able to work independently
  • Excellent organisational, communication, and computer skills
Desirable Skills
  • Experience in a similar case management role
  • Knowledge of iinsight, a cloud-based case management and reporting system
  • Advanced BABICM membership
  • Experience in care planning and managing care teams
