Clinical Case Manager

We are seeking a skilled Clinical Case Manager to join our Children and Young People team. As a key member of our team, you will be responsible for managing complex cases, developing and implementing individualized care plans, and collaborating with healthcare professionals to ensure the best possible outcomes for our clients.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals. You will also have a strong understanding of case management principles and practices, as well as experience working with catastrophic and complex injuries.
We offer a competitive salary of up to £56,000 per annum, depending on experience. As a self-employed or employed member of our team, you will have the opportunity to work on a flexible basis, with a range of benefits and support available to help you succeed in your role.
