  • Conduct thorough assessments of clients' needs, developing and implementing personalized care plans to achieve optimal outcomes.
  • Collaborate with healthcare professionals, therapists, and other stakeholders to ensure seamless care coordination and delivery.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure the best possible outcomes.
  • Provide education and support to clients and their families, empowering them to take an active role in their care and recovery.

Requirements:

  • Proven experience as a Clinical Case Manager, preferably in a catastrophic and complex injuries setting.
  • Strong knowledge of case management principles, practices, and regulations.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a high level of autonomy and flexibility.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
