About the Role

We are seeking a highly motivated and experienced Registered Nurse to join our dedicated team focused on complex discharge planning. In this crucial role, you will play a vital part in ensuring patients receive seamless transitions from hospital to home or other care settings.

Key Responsibilities

  • Provide expert nursing care and support to patients with complex needs throughout the discharge planning process.
  • Collaborate effectively with multidisciplinary teams, including doctors, social workers, and therapists, to develop individualized discharge plans.
  • Assess patient needs, identify potential risks, and implement strategies to ensure safe and effective transitions.
  • Educate patients and their families on post-discharge care instructions, medications, and available community resources.
  • Advocate for patients' well-being and ensure their voices are heard throughout the discharge process.

Essential Requirements

  • Registered Nurse with a valid license to practice.
  • Evidence of continuous professional development in relevant areas.
  • Post-registration qualification in your specialty or a willingness to pursue one.
  • Demonstrated expertise in providing nursing care across diverse clinical environments.
  • Proven experience in complex discharge planning and coordinating patient transitions.
  • Strong commitment to personal accountability for your practice and professional development.
  • Experience in formal and informal teaching, including educating patients and families.
  • Active involvement in service development initiatives.

Desirable Requirements

  • Bachelor's degree in Nursing or a related field.
  • Completion of a research module or experience in research activities.
  • Community nursing experience.
