Carer Support Specialist

We are seeking a compassionate and driven Carer Support Specialist to join our team. In this role, you will play a crucial part in connecting carers with the services and resources they need to thrive.

Your Responsibilities:
  • Provide personalized guidance and support to unpaid carers throughout their journey.
  • Facilitate the Carers Assessment process, ensuring carers receive comprehensive evaluations tailored to their unique needs.
  • Develop individualized Carer Support Plans that empower carers to take control of their well-being and effectively manage their caring responsibilities.
  • Maintain strong relationships with key stakeholders in the health and social care sectors to ensure seamless access to essential services for carers.
  • Promote awareness of carer support programs and resources within the community, connecting individuals with valuable opportunities for assistance and guidance.
Your Skills and Experience:
  • Proven experience in health or social care settings, demonstrating a deep understanding of the challenges faced by unpaid carers.
  • Exceptional communication and interpersonal skills, enabling you to build rapport with individuals from diverse backgrounds.
  • A collaborative approach, fostering strong partnerships with colleagues, professionals, and community organizations.
  • Proficiency in Microsoft Office Suite and database management systems.

Essential Requirements:

  • A valid driver's license and access to a reliable vehicle due to the city-wide nature of this role.
  • An enhanced Disclosure & Barring Check (DBS) clearance.
  • Completion of mandatory safeguarding training as required for this position.
