Main Purpose of the Role

As a member of the Local Solutions team, you will work closely with unpaid carers to provide them with the support and resources they need to maintain their health and wellbeing.

Key Responsibilities
  • Support carers through the Carers Assessment process, using a variety of methods to ensure they receive the best possible care.
  • Develop and implement Carers Support Plans, taking a person-centred approach to meet the unique needs of each carer.
  • Provide carers with information about the range of services available to them, and assist them in accessing these services.
  • Liaise with colleagues in statutory services to ensure seamless support for carers.
  • Work collaboratively with key stakeholders to raise awareness of carers and promote the referral pathway for carers.
Accountabilities
  • Ensure that all carers supported by the service are accurately recorded and followed up to ensure they have received good support.
  • Meet internal and external demands for information to demonstrate compliance with targets, standards, and statutory requirements.
  • Maintain a high level of customer focus and provide excellent support to all internal and external parties.
Additional Duties
  • Carry out any other duties as necessary or requested by the team.
  • Participate in professional development to improve relevant knowledge and skills.
  • Ensure that all activities of the project provide equality of opportunity.
Requirements
  • Experience of working in health or social care.
  • Experience of supporting unpaid carers or working with people.
  • Ability to work as part of a team, prioritise own workload, and manage own diary.
  • Technical expertise in Microsoft Office and Outlook.
  • Enhanced Disclosure & Barring Check and mandatory safeguarding training.
  • Travel required, with a car owner/driver essential for this city-wide post.
