Job Summary

The Clinical Care Specialist will utilize specialized knowledge and skills to provide healthcare autonomously to patients in care homes. The Clinical Care Specialist is responsible for independent clinical decision-making, including assessment and diagnostic skills. Additionally, they will refer patients to relevant colleagues or specialties as needed. The Clinical Care Specialist will collaborate with the healthcare team to meet patient needs and deliver high-quality care. The post holder must be a registered healthcare professional with an MSc in Advanced Clinical Practice or equivalent, as well as be an independent Non-Medical Prescriber.

The post holder will support healthcare services by providing additional patient access on Saturdays.

Main Responsibilities

Key Responsibilities:

  • Conduct proactive care in care homes through ward rounds and follow-ups
  • Create personalized care plans for residents
  • Collaborate with the multidisciplinary team on care planning
  • Manage complex client caseload
  • Maintain accurate documentation and patient records
  • Prescribe medications and treatments as per guidelines
