About the Role

We are seeking a highly skilled and dedicated General Practitioner to join our team at Wirral Community Health and Care NHS Foundation Trust. As a GP in our Community Intermediate Care Centre, you will play a vital role in providing high-quality patient care and contributing to the success of our team.

Key Responsibilities
  • Collaborate with our multi-disciplinary team to facilitate the safe and timely admission, rehabilitation, and discharge of patients in the Community Intermediate Care Centre.
  • Provide patient-centered medical oversight and support for the assessment, treatment, and management of patients on the Unit.
  • Maintain accurate electronic medical records and respond promptly to any changes in patients' clinical conditions.
  • Adhere to the Unit's escalation policy to ensure timely interventions and patient safety.
  • Make appropriate referrals to secondary care or other agencies when necessary.
  • Utilize lab diagnostics, including Microbiology, Haematology, and Biochemistry, as required.
Requirements
  • Current Registration with the General Medical Council.
  • Recognized General Practitioner Qualification.
  • Evidence of continuing professional development.
  • UK GP performers list inclusion.
  • Experience of older persons medicine/frailty.
  • Interpretation of laboratory results relevant to Primary care.
  • Experience of working independently in General Practice.
  • Experience of managing complex cases independently.
  • Understanding of the principles of evidence-based medicine and rational prescribing.
  • Ability to take a risk-based approach to support discharge planning.
