About the Role

We are seeking a highly skilled and dedicated General Practitioner to join our team at Wirral Community Health and Care NHS Foundation Trust. As a GP in our Community Intermediate Care Centre, you will play a vital role in providing high-quality patient care and contributing to the success of our team.

Key Responsibilities
  • Collaborate with our multi-disciplinary team to facilitate the safe and timely admission, rehabilitation, and discharge of patients in the Community Intermediate Care Centre.
  • Provide patient-centered medical oversight and support for the assessment, treatment, and management of patients on the Unit, working closely with the wider multi-disciplinary team to ensure comprehensive care.
  • Maintain accurate electronic medical records, respond promptly to any changes in patients' clinical conditions, and adhere to the Unit's escalation policy to ensure timely interventions and patient safety.
  • Make appropriate referrals to secondary care or other agencies when necessary, and utilize lab diagnostics, including Microbiology, Haematology, and Biochemistry, as required.
  • Foster a collaborative and supportive environment within the team, contributing to the overall success of patient care delivery and the discharging of patients back to their own home setting.
