We are seeking a highly skilled and compassionate General Practitioner to join our team at Wirral Community Health and Care NHS Foundation Trust. As a key member of our multi-disciplinary team, you will play a vital role in providing high-quality patient care and contributing to the success of our Community Intermediate Care Centre.
About the RoleThe successful candidate will be responsible for collaborating with our team of healthcare professionals to facilitate the safe and timely admission, rehabilitation, and discharge of patients in the Community Intermediate Care Centre. You will provide patient-centered medical oversight and support for the assessment, treatment, and management of patients on the Unit, working closely with the wider multi-disciplinary team to ensure comprehensive care.
Key Responsibilities- Collaborate with the multi-disciplinary team to ensure seamless patient care
- Provide medical oversight and support for patient assessment, treatment, and management
- Maintain accurate electronic medical records
- Respond promptly to changes in patient clinical conditions and adhere to the Unit's escalation policy
- Make appropriate referrals to secondary care or other agencies as needed
- Utilize lab diagnostics, including Microbiology, Haematology, and Biochemistry
- Current Registration with the General Medical Council
- Recognized General Practitioner Qualification
- Evidence of continuing professional development
- UK GP performers list inclusion
- Experience of older persons medicine/frailty
- Interpretation of laboratory results relevant to Primary care
- Experience of working independently in General Practice
- Experience of managing complex cases independently
- Understanding of the principles of evidence-based medicine and rational prescribing
- Ability to take a risk-based approach to support discharge planning
