Job Overview

Our Primary Care Mental Health Service is a pioneering initiative that has garnered national recognition for its innovative approach to mental health care. By integrating with GP practices and Neighbourhood teams, we ensure that individuals with mental health needs receive timely and effective support.

We are seeking a highly motivated and registered Primary Care Mental Health Practitioner to join our team. The ideal candidate will possess excellent communication skills, the ability to conduct robust clinical assessments independently, and a passion for delivering high-quality care.

Key Responsibilities

  • Collaborate with Mental Health Liaison Practitioners to facilitate mental health assessments in primary care and support GPs with joint decision-making regarding secondary care referrals, signposting, and transition of service users from secondary mental health services back to Primary Care.
  • Provide support, education, and problem-solving approaches for adults of working age, enabling them to manage mild to moderate mental health difficulties.
  • Work autonomously in a well-supported role, allowing for flexibility and dynamic decision-making while maintaining a focus on service user needs.

Requirements

  • Hold a full UK driving licence and have access to a vehicle, as independent travel is required to meet service needs.
  • Be able to work effectively in a fast-paced environment, prioritizing tasks and managing time efficiently.
