Position Overview

The Central and North West London NHS Foundation Trust is seeking a dedicated Community-Based Physiotherapy Specialist to join our Rapid Access Services. This role is integral to our 7-day community service, which includes three key pathways: Urgent Community Response, Discharge to Assess, and Virtual Ward.

Service Description

Our physiotherapy team collaborates with a variety of healthcare professionals, including occupational therapists, nurses, paramedics, social workers, and pharmacists, to ensure that patients receive timely interventions. The aim is to prevent unnecessary hospital admissions and to facilitate early discharges for patients who can recover more effectively in their home environment.

We also have a dedicated Urgent Response Falls pathway, which allows us to assist non-injured fallers promptly, working closely with external agencies to respond within a short timeframe.

Key Responsibilities

  • Engage in cross-functional work across the three pathways of the Rapid Access Service.
  • Provide immediate assessments and interventions for patients at risk of hospital admission.
  • Facilitate safe and timely discharges from local hospitals for patients requiring additional support at home.
  • Conduct thorough assessments in patients' homes to ensure their safety and optimal recovery.
  • Collaborate with multidisciplinary teams to deliver comprehensive care tailored to individual patient needs.

Qualifications and Experience

  • Degree or Diploma in Physiotherapy.
  • Registration with the Health and Care Professions Council (HCPC) as a Physiotherapist.
  • Previous experience in a similar healthcare setting, particularly with older adults and rehabilitation.
  • Experience in supervising junior staff and participating in service development initiatives.
