Position Overview

The Central and North West London NHS Foundation Trust is seeking a dedicated Community-Based Physiotherapy Specialist to join our Rapid Access Services. This innovative service operates seven days a week and encompasses three key pathways: Urgent Community Response, Discharge to Assess, and Virtual Ward. Our physiotherapists collaborate with a multidisciplinary team, including occupational therapists, nurses, paramedics, social workers, and pharmacists, to provide timely interventions that prevent hospital admissions and facilitate early discharges for patients recovering at home.

Key Responsibilities

As a vital member of the Rapid Access Service, you will:

  • Engage in cross-functional work across the three pathways: Urgent Community Response Team (UCR), Discharge to Assess (D2A) Team, and Virtual Ward (VW).
  • Provide immediate physiotherapy assessments and interventions in patients' homes to avert hospital admissions.
  • Conduct risk assessments and develop treatment plans for patients transitioning from acute care to home-based recovery.
  • Work closely with community neuro and respiratory teams, general practitioners, and acute hospitals to ensure comprehensive patient care.

Service Details

This role requires flexibility, as the service operates with varying shift patterns from 9 am to 10 pm. The successful candidate will have the opportunity to work autonomously while being supported by a dynamic and approachable team.

Qualifications and Experience

To be considered for this position, candidates must possess:

  • A degree or diploma in Physiotherapy.
  • Registration with the Health and Care Professions Council (HCPC) as a Physiotherapist.
  • Experience working in a similar healthcare environment, particularly with older adults and those with complex medical needs.
  • Strong communication skills and the ability to work collaboratively within a multidisciplinary team.
