Job Summary

We are seeking a highly skilled and compassionate Alcohol Care Team Nurse to join our team at North Tees and Hartlepool NHS Foundation Trust. As a key member of our Alcohol Care Team, you will play a vital role in providing high-quality care and treatment to patients with alcohol misuse issues.

Key Responsibilities
  • Assessment and Care Planning: Comprehensive assessment of patients with alcohol misuse issues, including physical, mental, and social health needs. Develop and implement individualized care plans to address these needs.
  • Advice and Support: Provide guidance and support to emergency department staff on admission criteria for patients with alcohol issues.
  • Liaison and Collaboration: Build strong professional relationships with healthcare professionals within and outside the hospital to ensure seamless care coordination.
  • Screening and Intervention: Screen patients for alcohol-related health and social problems, and provide appropriate interventions or referrals.
  • Case Management: Undertake individual case management to provide immediate and ongoing support to patients who are unsuitable or unwilling to engage in community services.
  • Education and Training: Deliver brief advice to patients and their families on alcohol-related harms, and provide training to Trust staff on screening and brief interventions.
Requirements
  • Knowledge and Expertise: Demonstrated knowledge and expertise in working with patients with alcohol misuse issues.
  • Communication and Interpersonal Skills: Excellent communication and interpersonal skills to work effectively with patients, families, and healthcare professionals.
  • Time Management and Prioritization: Ability to manage time effectively, prioritize tasks, and meet deadlines.
  • Professional Development: Commitment to ongoing professional development to stay up-to-date with best practices in alcohol misuse care.
