We are seeking a highly skilled and compassionate Alcohol Care Team Nurse to join our team at North Tees & Hartlepool NHS Foundation Trust. As a key member of our Alcohol Care Team, you will play a vital role in providing high-quality care and treatment to patients with alcohol misuse issues.
Key Responsibilities- To comprehensively assess patients with alcohol misuse issues, gathering information about their physical health, mental health, social issues, and alcohol history to develop a personalized care plan.
- To work collaboratively with senior nurses and other healthcare professionals to advise and support emergency department staff on admission criteria for patients with alcohol issues.
- To liaise with and build strong professional relationships with other healthcare professionals within the hospital and external agencies to meet the needs of patients.
- To develop therapeutic relationships with clients and provide individual case management to support those who are unsuitable or unwilling to engage in community services.
- To screen patients to identify those at risk from alcohol-related health and social problems and undertake appropriate interventions or referrals.
- To attend multidisciplinary meetings to discuss referrals, frequent attenders, or patients in treatment.
- To make accurate assessments of the risk patients pose to themselves and others and take necessary action within Trust safeguarding guidelines.
- To educate patients and their families on alcohol-related harms and deliver brief advice.
- To provide training to Trust staff on screening and brief interventions and participate in the development and delivery of education programs.
- RMN or RGN at degree level (or equivalent) with current NMC registration.
- Awareness of the need for a multi-agency approach to care planning for patients who abuse alcohol.
- Clinical knowledge of substance use and alcohol withdrawal.
- Ability to work within a multidisciplinary team and in partnership with other agencies.
