Staff Nurse

We are seeking a highly skilled and dedicated Staff Nurse to join our Alcohol Care Team at North Tees & Hartlepool NHS Foundation Trust. As a key member of our team, you will play a vital role in providing high-quality care and support to patients with alcohol misuse issues.
Key Responsibilities- To comprehensively assess patients with alcohol misuse issues, identifying their individual needs and developing effective care plans.
- To work collaboratively with multidisciplinary teams to provide comprehensive care and support to patients, including those with complex needs.
- To advise and support emergency department staff on admission criteria for patients with alcohol issues, ensuring that patients receive the best possible care.
- To liaise with and build strong professional relationships with other healthcare professionals, both within the hospital and external agencies, to meet the needs of patients.
- To screen patients to identify those at risk from alcohol-related health and social problems, and to undertake appropriate interventions or referrals.
- To undertake individual case management to provide immediate and ongoing support to patients who are unsuitable or unwilling to engage in community services.
- To make accurate assessments of the risk patients pose to themselves and others, and to take necessary action within Trust safeguarding guidelines.
- To provide training to Trust staff on screening and brief interventions, and to participate in the development and delivery of education programmes.
- RMN or RGN at degree level (or equivalent) with Current NMC Registration.
- Awareness of the need for a multi-agency approach to care planning for patients who abuse alcohol.
- Ability to work within a multidisciplinary team.
