Job Summary

We are seeking a highly skilled and compassionate Alcohol Care Team Nurse to join our team at North Tees & Hartlepool NHS Foundation Trust. As a key member of our Alcohol Care Team, you will play a vital role in providing comprehensive care and treatment to patients with alcohol misuse issues.

Main Responsibilities
  • Assess patients who misuse alcohol and develop individualized care plans to address their unique needs.
  • Collaborate with multidisciplinary teams to provide education and training on alcohol misuse and its effects.
  • Participate in the development and delivery of education programs for Trust staff on alcohol misuse and its treatment.
  • Support patients who are not engaging in community services and require ongoing support and care.
  • Attend multidisciplinary meetings to discuss referrals, frequent attenders, or patients in treatment.

Requirements
  • RMN or RGN at degree level (or equivalent) with current NMC registration.
  • Awareness of the need for a multi-agency approach to care planning for patients who abuse alcohol.
  • Ability to work within a multidisciplinary team and in partnership with other agencies.
