We are seeking an experienced and skilled Community Nurse to join our Havant Community Care Team at Southern Health NHS Foundation Trust.
As a Community Nurse, you will be responsible for providing high-quality care to patients with complex care needs in their own homes. You will work collaboratively with the team to manage a defined community caseload, ensuring that referrals are dealt with according to clinical need and that holistic assessments and care plans are recorded in line with Trust and professional standards.
The ideal candidate will have excellent clinical, managerial, and communication skills, with the ability to work autonomously and support the team lead in managing the case load and staff development.
You will be responsible for:
- Managing and coordinating work to ensure referrals are dealt with according to clinical need
- Recording holistic assessments and care plans in line with Trust and professional standards
- Providing high-quality care to patients with complex care needs in their own homes
- Working collaboratively with the team to manage a defined community caseload
- Supporting the team lead in managing the case load and staff development
