Community Health Specialist

We are seeking an experienced and skilled Community Nurse to join our Havant Community Care Team at Southern Health NHS Foundation Trust.
As a Community Nurse, you will be responsible for providing high-quality care to patients with complex care needs in their own homes. You will work closely with the team to manage a defined community caseload, ensuring that referrals are dealt with according to clinical need and that care plans are recorded in line with Trust and Professional record keeping standards.
The ideal candidate will have excellent clinical, managerial, and communication skills, with the ability to work autonomously and as part of a team. You will be proactive and positive in your approach to learning and development, with a commitment to improving patient outcomes and quality of care.
Key Responsibilities- Manage and coordinate a defined community caseload, ensuring that referrals are dealt with according to clinical need.
- Develop and implement holistic care plans, in line with Trust and Professional record keeping standards.
- Work with a high degree of autonomy and professional accountability, taking ownership of your caseload and making decisions that impact patient care.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Contribute to the development and implementation of care pathways and protocols, ensuring that they are evidence-based and effective.
